Sekai Project have announced that they are working with Very Ok Vinyl to release the soundtrack to Clannad on vinyl.
There are currently no further details on this release.

August 10th Memorial
For their 10th anniversary the visual novel game company August released a 3CD set with a selection of music from their different games. The limited edition of this set included a vinyl record with 6 tracks.
It is a single black vinyl LP.
It released on December 29, 2011.
Planetarian Analog Collector’s Edition
Key Sounds Label have succesfully funded a picture disc LP release for the visual novel and series Planetarian ~The Reveries Of A Little Planet~.
It was originally scheduled to release in September 2017, but due to a small production delay it released late October 2017 instead.
AIR Analog Collector’s Edition (Tori No Uta / Farewell Song)
A 12″ picture disc have been released with Lia’s songs from this visual novel. It was released by Key Sounds Lab.
